There is no offline mode for Google Forms. You can use Clappia, an alternative to offline Google Forms, to create form-based apps similar to Google Forms that can be used to collect and submit data offline.

I’m G2’s research analyst for the education industry — thanks for your question. As far as I know, there’s currently no way to use Google Forms offline. There is an offline mode for Google Sheets (see instructions here: https://support.google.com/docs/answer/6388102?co=GENIE.Platform%3DDesktop&hl=en), so you would be able to get offline access to a Google Sheet that holds the data captured by a Google Form. But the Google Form itself doesn’t work unless you have an internet connection.
