I assumed it was a lack of proper coding/development. Everyone wants to get something out of the door, so they don't perform sufficient testing. The sad thing is that there doesn't seem to be any quality control or improvement cycles. So people just stop doing the surveys cause they know they won't get to the the end of it anyways.

Very good question , and to me it is very unprofessional .I think it is a ploy the companies to weed out the survey participants actually.
